OK here is what the paper states.
H. TEST(S) REQUESTED BY EMPLOYER
[ ] 5 PANEL
[ ] 5 PANEL + ALCOHOL
[ ] 9 PANEL + ALCOHOL
Does this mean that i have to give to give 2 samples?
The container seal sticker at the buttom of the paper work has cut marks for 2 stickers and one side of the sticker states bottle A and the other bottle B (SPLIT). But they have the same serial number.
I plan on using the ureasample and is being shipped as we speak. Hopefully get here by tommoro. And hopefully they dont ask me to strip.
Any veterans out there have any input? GREATLY APPRECIATED

Its against the law for them to ask you to strip. You are not a criminal under arrest. You are taking a test for a job.
You won't be asked to give 2 samples unless you had been suspected of adulteration or submitting a sample outside of the proper temperature range (in such a case, the 2nd sample is submitted under direct observation).
In split-sample cases, one sample is submitted (45ml), then the tech splits it to 30ml for sample A, 15ml for sample B.

thanks toast sounds like i am good to go just have to worry about tempature range. I know they usually seal the sample. when you refer to the tech do you mean the people who actually test the sample.
Thanks for you input greatly appreciated.

To be a little more precise, I actually meant the sample collector.

well i went in and taped the ureasample under my armpit with heatpad. put in the sample and away it was sent off. I guess the test didnt require B (split) becuase she only put it in one sample cup and had me match the serial numbers. And i only initialed the A sample sticker. Thanks for the heads up. greatly appreciated. this forum is this shiznit

You only have to give them one sample....they split the sample after you give them your pee. Sample A is the one they actually do the test on.... Sample B they hold for up to a year in case you test positive and that is the sample that would be retested.
They are supposed to put the seals on in front of you and then you initial or sign the paperwork...trust me....there is a sample B..... I repeat....thats what they use to retest if you came up positive.
ALWAYS FORWARD.....NEVER STRAIGHT
