Generally, when comparing prices and speeds of DSL to dial up, I believe it's well worth it. You still have to use a phone line, but it won't block/busy the line like dial up. They give you filters for that, so you can rcv and make calls and surf the web at the same time.
I can't answer questions about packages, because that also varies by company. Check out the company's web site for DSL FAQs. However, unless they are offering a promotional deal (ie: free modem, first few months at reduced rates, etc.), I'd wait on it. Especially because high speed modems can sometimes be a bit pricey. But if you're fed up with dial up (which I was at the time), then go for it.
You can install it youself. Have them send you the equipment. Don't let them try to send over some fat ass hairy ignorant 'tech' do it for you. It will take you like 5 or 10 minutes. It's mainly just hooking up a bunch of cables and cords between your pc, the modem, and the phone jack.
