Arizona MM Card Questions

OK so i want to stop having to look over my shoulder when i am holding so i am looking to get my Medical Marijuana card. The reason i smoke is i have shattered 2 vertebrae, broke all the ribs on my left side and broke my right pelvic bone among other serious injuries as well.
So, having extensive medical records and obvious chronic pain all day every day, I tried smoking and it was much more effective in taking away the pain and i was still able to function with any issues. Because of these reasons, there's one thing i can say. Smoking helps a great amount. BUT
The one issue i have is my criminal record, so i was wondering if it would be possible under the listed circumstances if i could qualify for a medical card.
If i have had 2 drug possession charges and am on supervised probation, and also have had a disorderly conduct charge all of which are misdemeanors except the charge i am on probation for which is having narcotics(vicoden) without my script on me but it is undesignated so it will be misdemeanor after completion of probation.

So basically i am asking if i can get a medical card in phoenix AZ with my criminal record and being on probation.
