HIP Survey Seeks Community Input

Community Input Survey seeks additional feedback through July 2, 2010




The Health Improvement Plan (HIP) Committee has been hosting Community Listening Sessions to gather input and add a regional and community perspective and voice to its work. You are invited to complete this survey if you have not been able to participate in one of the community sessions.

Before completing the short, 3 question survey, please review the Listening Session Small Group Discussion Notes posted below. Please focus your survey comments on what's missing and providing your additional recommendations for the committee's consideration.

Pendleton - April 29th Madras - June 10th
Medford - May 13th Prineville - June 10th
Hillsboro - May 26th Bend - June 10th
Portland - May 26th Grand Ronde - June 24th
