The path to the med card

At the office I work in I get some of the same questions everyday so I thought I would post some imformation to help out people with the Nevada program.

First off are you a nevada state resident? This is a nevada drivers license or state id. Second no previous felony convictions related to the selling of a controlled substance. Third a qualifying medical condition under NRS 453A.050 that is properly documented. If you have these things go ahead and order your packet from the state. It takes a simple note, $50 check or money order, envelop and stamp and your on your way.

The state has been backed up some this year and its taking up to 30 days for people to receive their packets from the state. The packet itself has 5 forms that require attention. There is a registration form, couple notary forms, fingerprint card, attending physician statement that all need to be filled out. The state will need a copy of your NV license or id as well. A $150 check or money for the state to complete the packet as well.

So you have $200 to the state, notary fees, fingerprint fees and doctor fees. If your doctor will not sign the form yet you have a qualifying condition search for it on google and many doctors names will pop up that will sign but they do cost from $150 up.

Follow the instructions and get your packet completed and mail it back off to the state. There should be 7 things in the envelop: check or money order for $150 for the state, registration form, copy of id, waiver, acknowlegdement, fingerprint card, and attending physicians statement. After mailing it in you should receive a letter from the state and a receipt for the $150. If you do not receive anything after a month or so call the state office at 775-687-7594 and see if they sent it out. Wait the full month though as the girls up in Carson City are very busy.
