They do not pat you down for a Pre-Employment;that is my experience.
Only under very unusual circumstances such as court orders for custody disputes. Generally it is even forbiden by dept of corrections, & probation to search or accompany you in the restroom while you take the test.
No pat downs. They usually ask you to empty your pockets

But luvfriday mentioned court ordered for custody disputes... well, I happen to be in just one of those.
DSS is having me go to an outpatient drug/alcohol rehab facility to do this... small practice (from the looks of it)

They'll ask you to empty your pockets or some rare occasions ask you to life your pants up a bit to make sure you're not storing anything in your socks.
For employment-related tests, the collector may have you remove hats, jackets and emptying of all external pockets. The collectors are not allowed to frisk or otherwise touch the donor in any manner. This would constitute "groping" and is against the law.
