Getting a card with no medical records

Is it possible to get my 215 if I don't have any medical records of my problem? I am quite sure that I suffer from anxiety. When I was getting a physical about 5 years ago my doctor asked if I had anxiety, she said that I displayed all of the symptoms. But I denied it, because I do not believe in psychologists (went to one for 'anger problems' when I was young, complete BS) and didn't want to take any drugs. So there is no medical record of my condition. I have been using herb for 5 years to allieviate my symptoms, discovered that it was beneficial about 3 years after I started smoking recreationally. Will I be able to get a card?

I am in Humboldt county.
