I don't what kind of fan it is but I figure I will go ahead and tell you this. If the fan isn't intended to be on a controller it will fry the fan (pretty damn fast).
From what I gathered all you need to do is put the controller on the hot wire bringing power the fan.

What SmokeNChoke said is correct and it would appear that you have cut the black wire and inserted the control in the middle of it. Now, Hook the black wire from your cord to the black wire's lug, the white wire from your power cord to the blue wire's lug and the green wire from your power cord to the Yellow green wire's lug. (Of course I mean to connect to the empty lug adjacent to the wires.)
Give your wire connections a few real good tugs to make sure the connections you installed are tight and won't pull apart. If they are tight, wrap with black tape well onto the wire - you don't want the connection exposed like it is now. If they aren't tight and will pull apart, get rid of those crimp connectors and get screw on wire connectors.
Good luck! :thumbsup:

Actually, I think I earned the "Idiot" title here. :D I didn't see your power cord coming in and assumed you need to hook one up. So, ignore all the bullshit about hooking to the lugs and just check your connection as I suggested. You should be good to go. :thumbsup:

Black = hot
white = neutral
green = ground
To use the controler, have it break/make the hot for switching and control. the crimp on connectors are not worth the powder to blow them to hell if you don't know how to make proper crimps. Wire nuts (this is not a social disease) are a good alternative for connecting wires together.
