1) What medical marijuana doctors would you recommend for the Boulder area?
2) Do I need to send anything in with my temporary Colorado drivers like photo ID? If so would my old out of state license work?

the guy on 55th that has hours only on Saturdays is the only one I know of in/near Boulder that has been going long enough to trust. see boulder craigs list.

Yes you will need a photo ID to send them. Out of state will work with the temp paper one from Colorado. I would copy a bill too and send it. Any bill will do.
I let a MMC do mine and the dumb ass didnt put my paper license in the copy just my out of state license and guess what? They rejected it. My fault for not double checking his work. Dont rely on anyone to do the right thing but your self. Now i am looking at double the wait time and they are already behind so really looking at 3 x the amount of time it should normally take because he didnt copy my proof of residency that i gave him.
Make absolutely sure every letter and number is plainly written. Dot all your I's and cross all your T's. I am dead serious they have people there will reject application for one simple clerical error and some that wont. Guess which person you would get it if you had a clerical error on it.

When I renewed last Sept, I checked the wrong caregiver box and almost sent it in, fortunately I always double check before sealing the envelope. It was originally notarized in the doctors office and it was a Saturday so I had to find a notary & I ended going to the UPS Store (LOL)
I just recently sent in my application for indigent status on my card been almost 3 weeks, no new card yet, hopefully soon. I did get my discount card from MMAPA the other day & My current MMC (dispensary) will honor the discount on any strain in the store. Nice to be able to get a little help on a fixed Income.

One (1) of the following:
Colorado Driver’s License
Colorado ID
Temporary Colorado Driver’s License
Temporary Colorado ID
Or two (2) of the following:
Minimum of one (1) from the group of IDs below -
Out of State Driver’s License
Out of State ID
Passport, Military ID (copy of front and back), Tribal ID
And a minimum of one (1) from the group below -
Proof of Colorado Employment (paycheck stub/W-2)
Copy of a utility, medical, or cable bill. (The mailing address on all bills must match address on application. For utility and cable bills, the service address must be in Colorado.)
i. All documents must be currently valid when received at the Registry. Damaged, expired, or tampered IDs are not valid.
ii. Proof of residency materials must be current, within 60 days of the date the Registry receives your paperwork.
iii. At least one (1) of these documents must show the patient’s date of birth

Hands down fully alive medicine in Boulder are the best docs in the state. I will never go anywhere else ecspecially after being screwed by welldocs! check them out fullyalivemedicine.com ~Namaste

Thank you all advice is very helpful. I will give Fully Alive a shot this week and will send in out of state ID with Temp ID hopefully there will be no problems. Will let you know how it goes thanks again.

I know you asked about Boulder.
We have been using the MMD Medical Doctors in Denver for over 2 years and can't say enough good things about them. They are a full service doctor's office in addition to writing MMJ recs. I believe they 5 doctors on staff willing to write recs in addition to primary care physician work, personal injury, pain management, massage, acu, chiro, PT, etc. Worth giving them a call for sure. I made an appt with the boulder doc at 55th and arapahoe and when I walked in, it was completely filled 18 year olds. I promptly left and bought $75 of top shelf off my friend. :)
