UPDATE 2/14/11

RENEWING:
Applicants wishing to renew their registry cards are required to send in their renewal application no sooner than 60 days before the EXPIRATION DATE ON THEIR CARD. Applications received prior to this 60 day window will be considered a duplicate application, the applicantā??s payment will be cashed, and we will then require another $90.00 fee when renewing within the accepted time frame of within 60 days of the applicants current registry cardā??s expiration. We require a new application page, physician certification, $90 fee and copies of patient ID and/or caregiver ID for renewal. ID is not currently required if choosing a medical marijuana center as your provider.

PAYMENTS:
Payments sent to the Medical Marijuana Registry must be dated within the prior 12 months of the date of application submission. Payments which are dated older than 12 months will cause an application to be returned for a new payment. All payments made to the Medical Marijuana Registry are non-refundable and will be deposited within 2 weeks of application submission.

The Medical Marijuana Registry now cashes payments from both accepted applications and applications rejected and sent back to the applicant for correction as the fee for processing applications is non-refundable. Patients have 60 days to fully correct and return a rejected application from the date of rejection. If the applicant returns the fully corrected application after the 60 day window, an additional $90.00 fee will be required.

CUSTOMER SERVICE:
Requests for registry status updates, either by phone or email, must, by law, come directly from the patient. We cannot and do not respond to status requests from anyone other than the patient. This policy and procedure is to ensure the constitutional confidentiality right of the patient.

INDIGENCE:
The Medical Marijuana Registry accepts Supplemental Security Income (SSI) and Food Stamps as proof of indigence for the Fee Waiver program. We do not accept Medicaid, Medicare, SSDI or any other program beside SSI and Food Stamps.

For SSI we require that you submit a copy of your SSI award letter complete with SSI letterhead. For Food Stamps we require you submit a copy of your Food Stamp Eligibility determination letter. All documents must be valid at the time of application submission.
