What a day. Had to come home after the cat attacked one of the children, that was a trip. Then I was going through the mail organizer and I find the envelope that I sent in to the DOH back in December with my check and caregiver info, and it has a "Insufficient Funds" stamp on it :wtf:
Turns out that postage must have gone up somewhere along the line and I sent out this stuff with an old stamp I had. I feel stupid as hell. I mail, seriously, about 2 things a year, and they're usually packages. So what now? Do I have to go to another doctor? Do I just mail it back in? (It's from the 9th of December) Am I screwed? I'd like to call someone, but who?
It must have came back and the kids just put it up without thinking or something. It's not the end of the world but geez.
Any help guys?

not clear on what you mailed? the paperwork for change of caregiver? thats what it sounds like?or you paperwork for your card? if it's your card call your doc.

Check out this link for instructions. "The Registry must receive your complete application within 60 days of the physicianâ??s signature." When did the physician sign it? Send that sucker in ASAP, certified!

It was everything, the caregiver info and the stuff from the doc. It's been 54 days, so if I get it off Priority tomorrow, I'm thinking I'll be good?

(1) Kick the cat. (I can't stand the damm things. Gimmee a dog any-day)
(2) Hug the kid. Can't go wrong with a lil love!
(3) Mail the application... must do before step four. Remember to get a receipt!
(4) Now you can enjoy a bit O' the herb. You've earned it. :jointsmile:

At the end of the day, it's probably a good thing that the whole thing went down, or else I wouldn't have found this hiding away. I wonder what could have happened?

I also went to C3 about a week ago. The nice lady there said the notary only lasts something like 10 days. Check to see if this is correct.
If so, C3's notary only charges $5. If the notary is expired it's possible it could be denied. I'd hate to see you get it back again over a 5 min signature and $5.

If I'm not mistaken, your doc's recommendation has to be submitted w/in 60 days of it being signed. Sorry, responded before I saw the other posts.
