What do I need to give to my provider so that he can grow my medicine without repercussion?
I was thinking just a copy of my permit with a letter?

Yep. Just a copy of your authorization and a signed letter authorizing them to be your caretaker.

I ____________ declare under laws of purjury that________________ has my permission to be my designated provider for my medical marijuana.
This should work.
I would not get it notarized, that is not required by law.
Caretaker has been replaced by designated provider.

If you want to go even further you can have it notorized for a couple bucks. It just provides a witness that you both signed the document in there presence.

Thanks for the advice. My room-mate and long time Crohn's disease sufferer is authorizing me to provide for him. Is there anything else I can do to ensure my safety? What about when purchasing from the black market for him? Am I allowed to do that?

As provider/caregiver, you can grow and possess a certain amount of MMJ (varies by state). It is not legal to buy/acquire from someone else or use it yourself (unless you are also a legal user). What you do is up to you.
