Adrenaline Rush

Hi. I have some questions. I am trying to get my legal cannabis card but i'm not sure I completely understand on how to go about doing so.
I suffered from a Lumbar strain at my job back in 2002 and received medical treatment through Workmans Compensation. I continued to treat through 2003 and early 2004. Months after I settled my case (in which I was found to have permanent disability) I began smoking cannabis through a freind of mine. It actually helped ease the pain in my low back and to top it off it also helped with my migraines (I also suffer from migraines). So I never bothered continuing to treat for my low back injury from early 2004 through the present.
My question is, can I still get a physician's recommendation???
Or do I need to have updated medical records? Cause if that's the case I still have future medical benefits through workmans comp and I can go back for treatment anytime for my low back.
The problem is I don't have private health insurance and that's why I never bothered treating my migraines or else I would be treating my migraines. I simply buy prescribed medication from Mexico for my migraines.
I'm 2 hours away from the Oakland clinic so if I go up there I want to make sure I have everything I need.
So what do you guys think, do I have a shot and getting a physicians recommendation?
